hospital discharge support
Sometimes people need help and assistance when preparing for or just after discharge from hospital. This support package aims to meet this need.
Following a professional assessment by our care coordinator, a discharge plan will be set up and agreed usually for a time limited period. If it is practical support or personal care that is required then our service will provide a flexible package to support service users.
This support service aims to:
- assist discharge from hospital
- offer flexible care packages, always taking service users and family members wishes into account
- target services at those in greatest need of re-enablement
- provide support to carers when required
- provide support to complement other services as appropriate
